CCBHC

PermiaCare offers crisis intervention/suicide prevention services 24 hours a day, 7 days a week, 365 days a year.  These hotlines provide 24 hour a day contact for persons experiencing emotional crisis situations, including possible suicide.  The hotline is answered by an American Association of Suicidology (AAS) accredited hotline. Staff are available to provide face-to-face intervention including mobile assessments by our Mobile Crisis Outreach Teams (MCOT) and emergency hospitalization. 

PermiaCare provides open access to outpatient mental health services. If you are seeking routine mental health services, you may walk-in the mental health clinic

For initial visits, please bring:

• • Proof of identity (driver’s license, photo ID, Social Security card and identifying picture, etc)
  • Proof of income (last month of check stubs, Social Security Benefits Awards letter, letter stating you are providing financial assistance, etc.)
  • Proof of residence (utility bill, lease agreement, etc.)
  • Proof of insurance, Medicaid, Medicare, or another third-party payor (if applicable).

You will not be denied if you are unable to present the documents above.

• Invididuals are able to meet with a licensed clinician for their initial appointment
• Most PermiaCare clinics have psychiatric evaluation appointments available within 30 days from walk-in appointment
• Because of CCBHC certification, PermiaCare was awarded a SAMHSA CCBHC-Expansion grant in August 2021 which has allowed for PermiaCare to expand access to substance use services. PermiaCare now has Licensed Chemical Dependency Counselors (LCDC) and LCDC-Interns staffed in outpatient mental health clinics. These counselors are able to provide services that address co-occurring substance use and mental health needs in the same clinic where individuals meet with PermiaCare psychiatrists and staff.

• PermiaCare provides Veteran’s Services through a contract with the Department of State Health Services, providing peer support, advocacy, and linkage to other resources. Contact us at 432-770-9326 for more information.
• Mental Health First Aid training is provided to Veterans, Military Members, and their families to teach you how to identify, understand and respond to signs on mental illnesses and substance use disorders. The 8-hour training teaches skills needed to reach out and provide initial support to someone who may be developing a mental health or substance use problem and connect them to appropriate care.

• Care Coordination coordinates care across a spectrum of health services, including access to high-quality physical health and behavioral health care, as well as social services, housing, education systems, and employment opportunities as necessary to facilitate wellness and recovery of the whole person. Care coordination closes gaps in treatment by helping individuals access multiple types of services within the community and provides follow-up to ensure the individual starts treatment with those services to achieve their recovery goals. Care coordinators will link individuals to integrated health care programs, substance abuse programs, mental health services, veteran services, and any other additional services needed.
• Because of CCBHC certification, PermiaCare was awarded a SAMHSA CCBHC-Expansion grant in August 2021 which has enabled PermiaCare to be able to provide Care Coordination in all outpatient mental health clinics.

• PermiaCare staffs Certified Peer Specialists and Certified Family partners in outpatient mental health and substance use departments.
• Certified Peer Specialist are individuals who has personal lived experience in recovery from a mental health or substance abuse condition and who has been trained and certified to support their peers in gaining hope and moving forward in their own recovery journeys. PermiaCare now have four Certified Mental Health Peer Specialist.
• A Family Partner is a person who has lived experience parenting a child experiencing mental, emotional or behavioral health challenges and who can articulate the understanding of their experience with another parent or family member. This person may be a birth parent, adoptive parent, foster parent, legally recognized family member standing in for an absent parent, or a person chosen by the family or youth to have the role of parent. A Certified Family Partner has received specialized training and passed a Certification exam demonstrating that she or he has the competencies necessary to successfully navigate systems of care and help other families successfully navigate those systems.

Certification as a CCBHC was a process that began in September of 2018 which involved research to understand the national and state standards, substantial revision of many of our operating policies and procedures, an education campaign for staff culminating in our recent CCBHC University, and a six-month certification process administered by Texas and Health and Human Services. We are excited for the future opportunities being a CCBHC provides us to serve our communities and help those with mental illness and addiction issues.
